Insurance companies we participate with (are contracted with) include the following: United Concordia, Delta Dental- Advantage and Premier Plans, Metlife, Horizon BCBS- Traditional Plans, and Cigna. We do accept all other insurance plans as long as they are a PPO. HMO’s and Medicaid/ Medicare are not accepted at our office. Insurance Companies offer an array of different plans and benefits. Please contact your insurance company or HR department if you are unsure of your benefits. As a courtesy we may submit a Pre-determination to estimate you’re out of pocket responsibility. However, keep in mind that insurance companies only provide an estimate and not a guarantee of payment.
